Submission Guidelines

  • Cover note stating general description of operations, current and desired program, current carrier and target premium
  • ACORD Commercial Insurance Application and General Liability Application including appropriate supplemental application
  • Minimum of five years Historical exposures and loss information
  • Loss summary along with currently valued company/TPA loss runs covering a minimum of five years
  • Minimum five years historical revenues - use square footage or unit count if OL&T
  • Website information
  • Financials statements as appropriate
